Dairy in northwest Iowa to pay $38K for fish kill, manure spill in July

The Iowa Department of Natural Resources has ordered Roorda Dairy to pay over $38,000 due to a manure spill that killed over 107,000 fish in Mud Creek. Smithfield’s $2 million child labor penalty dwarfed by parent company’s annual $26 billion in sales

Smithfield will pay a $2 million penalty for child labor violations at its Minnesota plant, marking the state’s largest enforcement action. An investigation revealed that 11 minors worked hazardous jobs during late hours. The company denies knowingly hiring underage workers but has agreed to enhance compliance measures in response to the violations. Sioux City pharmacy fined for life-threatening medication error

A Sioux City pharmacy with a history of regulatory violations has been fined for a medication error that led to a life-threatening situation for a customer.

The Iowa Board of Pharmacy alleges that on Oct. 4, 2023, Greenville Pharmacy in Sioux City incorrectly filled a prescription for a customer who then began taking the drug.

Three weeks later, the customer was treated in the emergency room of a hospital and was then admitted for what the board says was a “life-threatening condition resulting from this medication error.”
