A three-vehicle accident involving an SUV, a pickup, and a semi-truck occurred in Woodbury County. The SUV failed to yield at a stop sign, resulting in a collision. The SUV driver and passenger sustained serious injuries and were taken to the hospital, while no other injuries were reported. Emergency personnel responded to the scene. In a recent case, an Iowa nursing home worker was fired for being honest with inspectors. The worker revealed issues the employer didn’t want disclosed, leading to immediate termination. Similarly, another worker was fired for allegedly instructing a colleague to lie to state inspectors. These incidents resulted in unemployment hearings, with some workers being awarded benefits while others were denied. State, city and county food inspectors have cited Iowa and South Dakota restaurants and stores for hundreds of food-safety violations in recent weeks, including poorly cooked beef, lack of food thermometers, unclean equipment, handling food with bare hands, repeat violations and dirty kitchens.
The findings are reported by the Iowa Department of Inspections, Appeals and Licensing, which handles food-establishment inspections at the state level. South Dakota food establishment inspections are handled by the South Dakota Department of Health. Listed below are some of the findings that stem from inspections at Siouxland restaurants and convenience stores over the past five weeks.

A nurse who allegedly failed to suction the airway of a U.S. Army veteran in an Iowa nursing home has been criminally charged in the man’s death.
Becky Sue Manning, 69, of Lake View, is charged with felony wanton neglect of a healthcare facility resident. According to prosecutors, Manning, a licensed practical nurse, refused repeated requests to provide physician-ordered care for a resident of the Fonda Specialty Care nursing home while working there in 2023 as an employee of the GrapeTree Medical Staffing temp agency.

The western Iowa man suspected of sexually assaulting an Iowa nursing home resident was the subject of a prior police complaint that “fell off the radar” without being pursued by authorities, according to police officials.The home’s parent company, Care Initiatives of West Des Moines, continued to provide work for the man in other Iowa nursing homes that it operates, inspectors found.
Seven months before the alleged rape, in March 2023, the suspect in the case was working through a temp agency at Friendship Home in Audubon when a female co-worker complained to management that the man had been making suggestive remarks to his female colleagues, sent them photos of himself masturbating. The Woodbury County Sheriff’s Office says it is still investigating the alleged rape of an Iowa nursing home resident who was subsequently evicted and dumped at a homeless shelter.

A news release from the Iowa Department of Agriculture and Land Stewardship Wednesday said that the agency and the United States Department of Agriculture (USDA) Animal and Plant Health Inspection Service (APHIS) have confirmed two positive cases of Highly Pathogenic Avian Influenza (HPAI) in Woodbury County, Iowa.
